PACE3: Influence Care Delivered in the Home

Welcome back to our PACE3 email series. In this installment we talk about one of the major issues facing PACE today, the lack of insight into what's happening at home. When participants are in our centers we know exactly what's happening. But when they leave, it's a mystery what happens in the home. Wouldn't it be great if you could influence what happens in those hours when participants are out of your sight?

Here's what we're hearing from you.

Today, current solutions include sending paper instructions home with the participant for the family caregiver. For example, some PACE organizations send home a paper calendar, which shows what care tasks should be completed when participants are home. Paper-based solutions depend on visual availability in the home, as well as the memory of the participant's family caregiver in the time of need. Further, these solutions offer no feedback for the IDT. There's no way to know if care tasks are being done.
